• 2. Complete and include a scanned copy of the ST-3 form (Sales Tax - Resale Certificate)*.
• 3. Complete and include a scanned copy of the ST-4 form (Sales Tax - Exempt Use Certificate) if applicable.

Application Status

• 8. You will receive an email notification once your application has been reviewed. This process typically takes 2-3 business days.
  Our Credit Department will contact you via phone or email with the account status.
